Required Immunizations for School Aged Children
Parents and Guardians:
Please see the attached immunization guideline for school aged children.
For those parents and guardians choosing to opt their child(ren) out of the required immunizations, the State of Oregon requires the following non-medical exemption module be completed for each vaccination.
